Updated Application Forms: For your convenience, DLLC has streamlined several liquor license application forms in addition to making them "fillable" using your computer. Revised application forms (below) can be identified by a 2009 date found in the lower left hand corner of the fist page. Old forms will be accepted until Monday, June 1, 2009. After 12am June 1, 2009, the old forms will no longer be accepted.
New Form Required For Application: Beginning at 12am April 1, 2009, The Arizona Statement of Citizenship and Alien Status For State Public Benefits Form, a Federal and State required form, must be completed by; a) statutory agents; b) individual owners including Joint Tenants With Right of Survivorship (JTWROS); and c) each partner (who is a person) in a partnership including general and limited partners. This does not include corporations that are partners. The Arizona Statement of Citizenship and Alien Status For State Public Benefits Form must be submitted with each new liquor license application and each liquor license renewal.
You will also notice an Audit Forms category below. These forms are designed to simplify Title 4 compliance for Hotel/Motel with Restaurant(series 11) and Restaurant (series 12) licensees. The new audit forms have been made "fillable" using your computer to expedite the audit process. The Business Data Report will be required with all Hotel/Motel with Restaurant(series 11) and Restaurant (series 12) liquor license renewals of businesses that have been in business for 12 months or longer. Welcome to the Deparment of Liquor License Renewal Forms On-line. For the convenience of our customers, the majority of Arizona liquor licensing and compliance forms are now available in a "fillable" format.

To download and use the forms you need:
An installed copy of Adobe Acrobat Reader 5.0 or higher. If you do not have this software, here's how to get a free copy. To view and print the forms, you will need the reader.

All forms must be printed on white bond 8 1/2" by 11" paper.
