Fingerprint Requirements
A.R.S. § 4-202, Qualifications of licensees; background information; prior convictions outlines application, background, and criminal history requirements for licensure to operate in Arizona. Subsection (B) specifies for the Department of Liquor Licenses and Control ("DLLC") to use the Department of Public Safety ("DPS") for processing its fingerprint samples.
General Information
Fingerprints and a Fingerprint Verification Form for each entrant are required at the time of submission of original application forms to DLLC.
Fingerprint samples, unless otherwise specified, are to be on the standard FBI APPLICANT (FD-258 (Rev 5-15-17) 1110-0046) cardstock. It is white card stock with light blue print, size 8 x 8 inches. Both the front and back of the card are required.

There is a non-refundable $22 fingerprint fee per card that is due at the time of application submission. The monies are a direct pass through from DLLC to DPS.
Arizona DPS Public Service Portal
The Arizona DPS will allow customers to submit requests electronically through a FieldPrint vendor. This will significantly reduce data entry backlogs and errors.Benefits
Applicants working directly through the DPS portal will be able to:- Individually send in their fingerprint-based background check application.
- Check the status of their background check on their "Dashboard".
- Receive individual background results, rather than waiting for a group or sheet to be returned.
- Receive background results electronically via the agency "Message Center".
- View the applicant's background status on the agency's "Dashboard" in one streamlined area.
Customer instructions for setting up the DPS Public Services Portal account:
In order for the applicant to complete the background check process, please follow the below instructions:- Individuals must create an account on the portal by navigating to the DPS Public Portal.
- Click the "Sign Up" link in the upper right portion of the screen. Then you will be given a choice of what account to create.
- Select the "Background Check for employment/licensure" icon.
- Answer "YES" to the next question.
- Select the "Individual" option. (PLEASE NOTE, IF YOU ALREADY HAVE AN INDIVIDUAL ACCOUNT, YOU DO NOT NEED TO CREATE ANOTHER ACCOUNT, YOUR EXISTING ACCOUNT CAN ACCESS THE NEW BACKGROUND CHECK FEATURE).
- Complete the fields. (you MUST ensure to input your full legal name as it appears on your government-issued identification. Any other name may delay their process).
- You will have to validate your email address by clicking on a "Verify Email" link sent to your registered email address.
- When asked, the Department of Liquor fingerprint code is: LIQ-007.A
- Once you enter your valid fingerprint code, you will be taken to the rest of the application which is your personal information and a few questions. When you complete this process, you will be taken to the "My Order" page to pay for your background fee by selecting SUBMIT AND PAY.
- Once the background check is submitted, you will receive instructions in your message center for the next steps on fingerprinting. You can elect to do fingerprints electronically through the DPS vendor, Fieldprint (https://fieldprintusa.com/individuals). You will need the reference number that was given to you in your message center and follow the instructions included.
